If you use text-to-speech-software, please disable the Flash Plugin (e.g. in Firefox in add-ons / plugins / select "never activate" for Shockwave Flash) and reload the questionnaire by pushing the enter key in the address bar. After this step you can enter your access code to start the questionnaire.
